FREEDOM OF CHOICE

We often refer patients to other medical providers and suppliers such as orders for laboratory tests or prescription drugs. You have the freedom of choice to use any willing provider or supplier for these services. However, sometimes we have negotiated reduced rates with select providers or suppliers and these reduced rates may only be available from selected providers or suppliers and the staff will let you know about your options. If you have any questions, please talk with your patient care representative or practitioner.
